Student-Athlete Questionnaire

All student-athletes should complete a questionnaire from the Sports Information Department every year that they participate in their respective sports. To make this accessible to you, we have placed this form online so that you might download it and complete it at your convenience. Please return this form to the Sports Information Office, located at the south end of the Fieldhouse near the football locker room.

Information you disclose on this form will aid us in the website updates, media guide/game program production, and on-air commentary for radio/TV/internet broadcasts.
